FDA Changes BabyBIG Lot 8 Dose and Infusion Filter
FDA has issued Lot 8-specific BabyBIG instructions reducing the dose to 20 mg/kg and replacing the previously supplied 18-micron filter with a 0.2-micron filter.
The U.S. Food and Drug Administration issued an important medication-safety communication on October 23, 2026 changing the dosage and administration instructions for BabyBIG Lot 8, the human botulism immune globulin used to treat infant botulism caused by toxin types A or B in infants younger than one year. [1]
For Lot 8 only, FDA recommends:
0.4 mL/kg, equivalent to 20 mg/kg, as a single intravenous infusion.
That recommendation supersedes the 1.0 mL/kg, or 50 mg/kg, dose in the existing prescribing information when Lot 8 is being administered. [1]
FDA has also changed the administration filter supplied with this lot.
The 0.2-micron sterile disposable filter shipped with Lot 8 should be used in place of the 18-micron filter supplied with previous BabyBIG lots. [1]
FDA says production changes resulted in higher antibody titers against botulinum neurotoxins types A and B than historically observed.
The higher antibody titers, together with efforts to maintain an adequate supply of Lot 8, support the lower Lot 8 dose. [1]
This is therefore not an FDA response to a BabyBIG overdose, dosing-error cluster or newly identified toxicity.
The previous prescribing-information dose should not be characterized as unsafe.
The new recommendation reflects the characteristics of Lot 8.
The administration change is not limited to the amount infused.
FDA states that the new 0.2-micron filter supplied with Lot 8 replaces the 18-micron filter that accompanied previous lots. The agency says comprehensive filtration studies support use of the new disposable filter. [1]
For hospitals, this makes lot verification important at both the prescribing and pharmacy-preparation stages.
The safest operational approach is to use the dose and administration materials supplied for the actual lot in hand, rather than assuming instructions from a previous BabyBIG shipment still apply.
BabyBIG, or Botulism Immune Globulin Intravenous (Human), is manufactured and distributed by the California Department of Public Health.
FDA lists it for treatment of infant botulism caused by botulinum toxin types A or B in patients younger than one year. [2]
The California Infant Botulism Treatment and Prevention Program currently instructs pharmacists to follow the updated dose information provided with BabyBIG shipments and lists 20 mg/kg (0.4 mL/kg) as the current recommended administered dose. [3]
For detailed clinical background on recognition and treatment, see Infant Botulism: Symptoms, Diagnosis, BabyBIG Treatment, and Formula Recall Safety.
MedicalToxic also has a relevant clinical example in Case Study: An 8-Week-Old Infant with Weak Cry, Hypotonia, and Respiratory Distress.
The key safety distinction is simple:
BabyBIG Lot 8 ≠ a universal new dose for every BabyBIG lot.
The FDA communication specifically addresses Lot 8.
Clinicians and pharmacists should therefore verify the lot, use the Lot 8-specific dosage information when applicable, and use the supplied 0.2-micron filter rather than substituting administration instructions from an earlier lot.
The Infant Botulism Treatment and Prevention Program remains available around the clock for clinical consultation and BabyBIG coordination. [3]
The important October 23 development is not a universal change to infant-botulism therapy.
It is a lot-specific change in both dose and infusion hardware:
Previous prescribing-information dose: 1.0 mL/kg (50 mg/kg)
BabyBIG Lot 8: 0.4 mL/kg (20 mg/kg)
Previous supplied filter: 18 micron
Lot 8 supplied filter: 0.2 micron
That combination makes this a high-priority medication-safety update for pediatric hospitals, pharmacists, intensive-care teams and clinicians treating suspected infant botulism.
FDA's communication does not state that the previous BabyBIG dose caused toxicity.
It also does not establish that the 20 mg/kg dose should be used for previous or future lots without lot-specific instructions.
No independent dosing extrapolation should be made from the Lot 8 notice.
FDA's October 23 communication changes two operational details for BabyBIG Lot 8: the recommended dose is now 20 mg/kg (0.4 mL/kg) as a single IV infusion, and administration should use the 0.2-micron filter supplied with the shipment. [1]
The change is supported by higher antibody titers associated with production changes and supply considerations.
The most important practical message is therefore not simply “BabyBIG has a new dose.”
It is:
verify the BabyBIG lot before selecting the dose and administration filter.
