ToxSOAP is designed for poison center specialists, medical toxicologists, emergency and ICU clinicians, hospitalists, trainees, and pharmacists who document toxicology consultations and poison center calls.

At minimum: patient age/sex/weight (if available), exposure intent and route, substances involved (or "unknown"), timeline (time of exposure and arrival when known), symptoms/exam findings, vitals, electrocardiogram summary, and any available labs or drug levels.

ToxSOAP is primarily a documentation and note-generation tool. If you include suggestion fields (assessment/plan prompts), they should be treated as clinician-authored content. Final interpretation and treatment decisions remain the responsibility of the treating provider using clinical judgment and local protocols.
